IU/PaTTAN Transition Conference Call Notes:

PaTTAN will be launching a new website that will replace the Secondary Transition Folders that you have been mailing to students. This information will be shared at the PDE conference that is being held in March. All of the hardcopy and CD materials will be housed on the new website for parents to access. A letter will be mailed to each LEA that can be shared with parents regarding how to request hardcopies of the materials for those that do not have means to access this information electronically.

Sheree's tidbit: Consider hosting a Transition Night for those parents of students that will be or are 14 years of age to access the website in you school computer lab. This would be a great activity to get parents thinking about the transition process and to connect with the point person for transition in you perspective locations.

There will be a webinar on April 17th (time to be determined) on how to navigate the website. STAY TUNED!!!

This site is designed to help you teach 6-12 graders how to manage money wisely. The need for such practical instruction is a serious one. Both test scores and the current money management behavior of young people indicate that they lack basic money management skills they need to learn as they grow up.

For the first time in this country there is a generation of youth with special health care needs, chronic health conditions and disabilities who have survived beyond their diagnosis / prognosis. For success in the classroom, in the community and on the job, young people with special health care needs must optimize their health potential. This requires an understanding of their health needs and involvement in their health care decision making. Access to quality care as youth become adults will require transition from child centered to an adult oriented systems of care.

In response to these issues the Maternal and Child Health Bureau’s Division of Services for Children with Special Health Care Needs (MCHB/DSCSHN) www.mchb.hrsa.gov has funded the development and demonstration of model Healthy & Ready to Work (HRTW) state programs focused on of children and youth with special health care needs (CYSHCN).
